After last week, Monday July 1 was an example of how it ought to work. No cancellations. No short workings.
Some late running - 1P28 1157 Great Malvern to London Paddington arrived +23. A few others +10.

Well, neither of the daytime Herefords got there today.
1W02 1152 London Paddington to Hereford arrived Great Malvern 23 late and was cancelled therafter.

Earlier today:

1P22 0856 Great Malvern to London Paddington departed 21 late as a result of a late-running West Midlands train. Further delayed en route, it arrived at Paddington 48 late.

As a result of this, 1W15 0750 London Paddington to Great Malvern was held at Evesham for 20 minutes.

And in the afternoon:

1W31 1552 London Paddington to Great Malvern was held at Evesham for 38 minutes. Was this the original "disturbance on a train"? It arrived Shrub Hill 39 late and was cancelled thereafter.

Behind it, 1W33 1658 London Paddington to Great Malvern ran to time, arrived Worcester Shrub Hill 3 late and was cancelled thereafter.

1W03 1734 London Paddington to Hereford left Oxford 9 but arrived Shrub Hill 25 late and Hereford 21 late.

The last two trains from Paddington are going round via Stroud this week.

And today, the second Hereford (1W02 11:52 from London Paddington) left 29 late, arrived Great Malvern +45 and was cancelled thereafter "due to a problem with the train (M8)" (according to RTT» (Real Time Trains - website)).

15:23 London Paddington to Worcester Foregate Street due 17:47 will no longer call at Hanborough, Charlbury, Kingham, Moreton-In-Marsh, Honeybourne, Evesham, Pershore and Worcestershire Parkway Hl.
It has been delayed at London Paddington and is now 19 minutes late.
This is due to a late running train being in front of this one.

According to RTT, it left Oxford +24, seems to have stopped at Hanborough and Charlbury, and is now only +8 at Evesham.
Difficult to identify the "late running train being in front of this one" - probably down to the inbound train, from Cheltenham, arriving +23.
