Despite what the article says, these are fines, compensation and costs and not fees. The level also suggests that they did not bother to respond to the court or did not supply details of their income.

They may have done ... but doesn't "the railway" have the choice of issuing a fixed penalty notice or taking someone to court if it believes they were intentionally travelling without a correct ticket?

I agree, they obviously ignored the fixed penalty notice

They may have done ... but doesn't "the railway" have the choice of issuing a fixed penalty notice or taking someone to court if it believes they were intentionally travelling without a correct ticket?

They do, and usually save this for the fraudulent use of things like season tickets. 

The fixed penalty automatically add a surcharge for travelling without a ticket.  TfL» (Transport for London - about) for example do not give you the option to buy a ticket on board a train they just issue fixed penalty
