So, after a train "overshot" the platform at Pewsey in October this year and the event ended up on the Jeremy Vine show, a similar thing happened again yesterday evening on the 1733 from Paddington. This time the driver apparently just "forgot to stop" and trundled through the station without comment. Someone eventually realised what had happened and we went to Westbury - thanks! Luckily there was a return train in around 20 minutes so not a massive problem but what about the people hoping to join the train at Pewsey?

How does this happen?    Huh

The joy of irregular calling patterns ... where such things happen, it's usually on a line / with a driver who doesn't normally stop at a particular station; they're still following signals so there's no safety concern.  The "Berks and Hants" seems to have just about the most erratic call pattern in GWR (Great Western Railway) territory, so statistically it's more likely to happen there than elsewhere.   It's also been known to happen at Chippenham; drivers based in Wales are familiar with using the line via Chippenham and Bath Spa when the line via Hullavington is closed, but on those diverted services they don't stop.   When they find themselves driving the 05:18 Paddington to Swansea ...

Tywford and Maidenhead are two other ones that quite regularly trip drivers up.  Culham had a very bad record as well but is a little better recently.  It would be quite easy for the industry to develop a warning system of some sort for the stations most commonly missed, but they don't seem interested in doing so.  The result is usually at least one such incident a month - and that's on GWR (Great Western Railway) alone.

Culham had a very bad record as well but is a little better recently.

South Wales (Swansea) use the line on a daily basis not just when the Hullavington route is closed.  They wouldn't be working the 0518 from Paddington though at that time of day.  How would they get to London ?
