If one is required to buy a ticket on a train and then find that one in unable - or unwilling in case of a dispute - what is the name of the form that the train manager/guard/ticket inspect serves upon you once you give your name and address?

The form differs depending upon whether it is simply an unpaid fare or whether it is a penalty fare.
In the case where the guard is satisfied that no penalty should be applied, it will be an 'Unpaid Fare Notice' (
UFN‡) or if a penalty is to be applied, then it will be a 'Penalty Fare Notice' (PFN).
The amount to be paid will be stated on the notice and the customer should take it to a ticket office to be paid.
