Mistakes happen.  They are more likely to happen at places such at Pilning with a very limited service as drivers are used to flying through and not stopping on a regular basis.

A driver under instruction would take no blame at all, but the incident would go on the Driving Instructor's record if it was their fault.  It would be considered 'their fault' if they have been issued with a diagram listing the call at Pilning or a special stop order for Pilning and they subsequently didn't stop, unless there are technical reasons (such as wheelslip) directly to blame.

Some TOC (Train Operating Company)'s have a different incident record for Driving Instructor's when they are instructing from when they are driving alone, as a different and more demanding skill set is required, but GWR (Great Western Railway) operates a single incident record procedure.
