From FGW (First Great Western) website

18 July 2008
Train services between Newport South Wales and Bristol Temple Meads via Severn Tunnel Jn are being disrupted due to signalling problems in the Severn Tunnel Jn area.Engineers are working as fast as possible to restore services to normal. Short notice alterations, cancellations and delays of up to 60 minutes can be expected.

Regret only a limited number of trains are able to transit Severn Tunnel Jn. Network Rail are working as fast as possible to rectify the problem.

No indication if this is still extant at 11.40, but trains were being terminated at Parkway earlier I believe.

This didn't seem to affect me too much when passing through the area around 3pm

Think it may have put some stock in the wrong place as the 16.22 off BTM (Bristol Temple Meads (strictly, it should be BRI)) was only 2 car which as you can image was absolutely heaving with passengers left at Bath.

Fortunately a 158, understand the earlier problem was an emergency pull self initiating.
