Cancelled mid-journey because the PA▸ (Public Address (broadcast loudspeaker announcements) or Passenger Assist (railway staff providing physical assistance to passengers with mobility issues), depending on context) wasn't working? Sounds like the safety Taliban were working overtime.

Well you could class the PA as a bit of safety equipment, it will become vital in the event of an incident.

It is classed as an 'out of service at the first convenient location' fault I believe. Maidenhead would fit that as it could be emptied and taken into sidings. Though, in my opinion, it is a bit of a over reaction. Quite right, Ollie, it could be very important in the event of an emergency, but a driver on a DOO▸ (Driver-Only Operation (that is, trains which operate without carrying a guard)) turbo has no idea at any time whether all speakers are working effectively in all carriages - only that the system is either active or faulty - so if it's that important why isn't it subject to a full test before every departure?!
