When they end up in reverse formation where in London are FGW (First Great Western) able to turn their HSTs (High Speed Train (Inter City class 43 125 units)) ?

They'll usually run from Paddington or Old Oak Common out to Greenford via Park Royal and back via West Ealing.

What tends to cause a reverse formation on the GW» (Great Western - used as an abbreviation for the area / lines under the Great Western franchise, as opposed to FGW which includes "First", the company operating them too. For tickets - about) mainline?

The first and last services of the day that run from Paddington to Cardiff via Bristol do it automatically!

Gloucester?

Yes but normally most FGW (First Great Western) Services go on to Cheltenham Alstone sidings which has the effect of turning the set back again

Keeping up with the ls the Llandore triangle. Non use of which contributed to the Southall crash by not having operational AWS (Automatic Warning System - an arrangement of permanent magnets and electro-magnets that convey information about the associated signal to the train driver) in the front cab.

Presumably running the circle Padd to Hereford via  Worcester and onto Bristol via Maindee. Also Exeter out via Yeovil back via Taunton.

Lots of possibilities

Indeed. Though there's less of a pressing need to reverse sets now that Selective Door Operation has been added to the CDL (Central Door Locking) system.

As does the 18 03 Pad to Frome evening service

This service is booked via it's diagram to be turned at the end of the day so will be in the correct formation when arriving at the Marsh! as is 1T88 Cheltenham to Bristol HST (High Speed Train (Inter City class 43 125 units))
