A rescue operation started just after 16:00 GMT involving police, fire, coastguards and RNAS CuldroseA body has been recovered after a car rolled off a slipway into the River Fal in south-east Cornwall.
The car slid into the water at about 16:00 GMT at the Roseland Peninsula, near Truro, where the King Harry Ferry is based.
It was retrieved two hours later. Devon and Cornwall Police said the driver escaped but an elderly woman remained trapped and died.
The operation involved the police, fire, coastguards and RNAS Culdrose.
The BBC's James Hoxey, at the scene, said staff from the ferry company raised the alarm after a big splash was heard and the car was seen sinking rapidly.
A spokesman for Devon and Cornwall Police said the rescue was more difficult than had at first been anticipated and involved police divers.
A rescue helicopter from RNAS Culdrose and inshore and all-weather lifeboats from Falmouth were also involved, as well as a civilian diver who inspected the car.
A statement from the King Harry Ferry said the car went into the water at about 16:00 GMT, when the ferry was at the Feock side of the river.
The King Harry Ferry connects St Mawes and the Roseland Peninsula with Feock, Truro and Falmouth and is an alternative route to a 27-mile (43km) journey through Truro and Tresillian. It has been operating since 1888.
