When the driver doesn't know/forgets and it ends up off the platform.

The Saturday morning through service to Newquay from Paddington has an extra standard class carriage marshalled between Coach H and the rear power car to accommodate solely peoples' luggage.

Everyone dutifully piles their bags, bikes, buggies and surfboards at the London end of Platform 4 at Reading only for the train to stop at the usual point with the extra coach off the end.

Result?  Get the passengers on. Drop the locks. Move the train forward a carriage length and then load the luggage.

Meanwhile the following trains diverted round it using platform 8.

As an aside who is senior despatch staff or train manager?

Train Manager in charge of safety of the train
Dispatcher in charge of safety of the platform.

The dispatcher would start the dispatch process.
