They'll usually run from Paddington or Old Oak Common out to Greenford via Park Royal and back via West Ealing.

What tends to cause a reverse formation on the GW mainline?

The first and last services of the day that run from Paddington to Cardiff via Bristol do it automatically!

Gloucester?

Yes but normally most FGW Services go on to Cheltenham Alstone sidings which has the effect of turning the set back again

Keeping up with the ls the Llandore triangle. Non use of which contributed to the Southall crash by not having operational AWS in the front cab.

Presumably running the circle Padd to Hereford via  Worcester and onto Bristol via Maindee. Also Exeter out via Yeovil back via Taunton.

Lots of possibilities

Indeed. Though there's less of a pressing need to reverse sets now that Selective Door Operation has been added to the CDL system.

This service is booked via it's diagram to be turned at the end of the day so will be in the correct formation when arriving at the Marsh! as is 1T88 Cheltenham to Bristol HST
