A sentence caught my attention
"A Network Rail spokesperson said: “A train passed a signal at danger and came to a stop over Sandwich level crossing on Tuesday."

Should be a "never happens" event.  But it's happened (several years ago) at the Littleton & Badsey CCTV supervised barrier crossing near my home, too
